This AFFILIATION AGREEMENT (the "Agreement"), is made this August 26, 2010, by and between the ACADEMY OF MANAGED CARE PHARMACY (AMCP), a 501(c)(6) nonprofit corporation, incorporated in the state of New Jersey, with its principal place of business at 100 North Pitt Street, Suite 400; Alexandria, VA 22314, and the Utah – Academy of Managed Care Pharmacy (UT- AMCP) ("AFFILIATE"), a/an unincorporated group, with its principal place of business in the State of Utah.
