This PHARMACY AND NURSING SERVICES AGREEMENT (the "Agreement") is effective as of the 8 day of June, 2004 ("the Effective Date") by and between M T Ultimate Healthcare, having its principal place of business at 45 Main Street Brooklyn, NY and a branch office at 2434 Grand Avenue Baldwin, New York ("Agency"), and The PromptCare Companies having its principal place of business at 51 Terminal Avenue, Clark, New Jersey, 07066 ("PromptCare").
