This Animal Foster Care Agreement is entered into between the Oklahoma Humane Society (OK Humane), a 501(c)(3) nonprofit corporation, and the undersigned (the “Caretaker” or “you”). Please read the entire agreement, and be sure you understand and agree with the conditions and restrictions provided below before signing. The terms “Caretaker” or “you” refer to all individuals over age 18, living at the address indicated at the end of this contract. You certify that you have never been convicted of animal cruelty, neglect, or abandonment at any time in any state. The Caretaker represents that no member of the household has ever been charged with cruelty to animals. In consideration of the mutual promises in this Agreement, and other good and valuable considerations, OK Humane and the Caretaker agree to the following terms and conditions intending to be legally bound.
