I, the undersigned subscriber, do hereby irrevocably undertake to subscribe to the DEUKOM Media Programme. I agree, subject to the conditions of contract reflected overleaf, to pay DEUKOM a monthly fee of R549,00 for premium subscription,by way of a monthly debit order or an annual payment of R6.039,00 + R159,- connection fee = R6.198,00.
