APPLICATION FOR USE OF FACILITIES PERMISSION MUST BE SECURED BEFORE ANY DEFINITE ANNOUNCEMENT IS MADE CONCERNING PROPOSED USE. A CERTIFICATE OF INSURANCE PROVING LIABILITY COVERAGE OF AT LEAST $1,000,000 PER OCCURRENCE/$2,000,000 AGGREGATE, NAMING THE TICONDEROGA CENTRAL SCHOOL DISTRICT AS AN ADDITIONAL INSURED BY USING AN ENDORSMENT, IS REQUIRED (SEE ATTACHED INSURANCE AGREEMENT FOR LIABILITY INSURANCE REQUIREMENTS). CERTIFICATE OF LIABILITY INSURANCE MUST BE SUBMITTED WITH THIS REQUEST; NO USAGE WILL BE APPROVED WITHOUT LIABILITY INSURANCE. The undersigned is over 21 years of age and has read this form and attached regulations and agrees to comply with them. He/she agrees to be responsible to Miner Institute for the use and care of the facilities. He/she, on behalf of the above stated organization does hereby covenant and agree to defend, indemnify and hold harmless Miner Institute from and against any and all liability, loss, damages, claims, or actions (including costs and attorneys fees) for bodily injury and/or property damage, to the extent permissible by law, arising out of or in connection with the actual or proposed use of Miner Institute’s property, facilities and/or services by the above stated organization.
