This BUSINESS ASSOCIATE AGREEMENT AND Data Use Agreement (“Agreement”) is entered into and made effective on ______________ (the “Effective Date”), by and between the Palliative Care Quality Collaborative, NFP, an Illinois not-for-profit corporation, with its principal place of business at 8735 W Higgins Rd, Ste #300, Chicago, IL, 60631 (“PCQC”) and _____________ (“Participant”). PCQC and Participant are each a Party to this Agreement and are referred to collectively as the “Parties.”
