In consideration of being permitted to participate in the activities of Detroit City Futbol, I, on behalf of myself and my heirs, represent, warrant, and agree to the following: I do not have any health‑related conditions, disabilities, etc.,that would restrict my participation in Detroit City Futbol activities. I understand the risks of participating in Detroit City Futbol activities and that participation in Detroit City Futbol activities can result in bodily injuries or even death. I hereby accept and assume the risk of and full responsibility for any loss, property damage, personal injury or death, which may be sustained as a result of my participation and involvement in Detroit City Futbol activities.
