I, (“I” or “Event Participant”), am a student at or otherwise affiliated with Dallas Baptist University, a Texas not-for-profit corporation (“University”). In consideration of the University’s agreement to allow me to participate in the DBU Explore Program, which participation is voluntarily, I agree to the terms of this Assumption of Risk, Indemnification, and Medical Authorization Agreement (the “Agreement”).
