I, , desire to participate in the ultra-hazardous activities defined in the following provisions (“Activities”) offered by Liberty Mountain Snowflex® Centre (“Snowflex”). In consideration of my being allowed to participate in the Activities, I understand that by signing this agreement (“Agreement”) I am assuming all risks of the Activities, including those specifically identified in the following provisions:

Before I may enter or participate in any activities within or sponsored by the Liberty Mountain Snowflex® Centre (“Snowflex”), I understand that I must read and promise to be bound by the following terms. This agreement is legally binding and can affect my ability to recover for injuries I may sustain. Accordingly, I agree by signing below that I have thoroughly read each of the provisions of this agreement and understand the entire agreement.
