This assumption of the risk and waiver of liability agreement (the “Agreement”) is executed by the Participant above, and, if applicable, his/her parent or legal guardian (below), in favor of C.S. Lewis Academy and Newberg Open Bible, Oregon nonprofits (“CSLA/OB”). The undersigned are aware and understand that the Participant will be participating in a program run by CSLA SUMMER SPORTS CAMPS (the “Program”), on property owned by Newberg Open Bible and managed by C.S. Lewis Academy.
