THIS SERVICES AGREEMENT (this “Agreement”) is made and entered into on the 16th day of July, 2018 (“Commencement Date”) by and between Fairview Health Services doing business as Fairview Northland Rehabilitation Services (“Fairview”), a Minnesota nonprofit corporation, and Independent School District No. 912 (“District”), contracting on behalf of Milaca High School (“School”) for a one-year period, 2018-2019 school year (the “Term”).

THIS SERVICES AGREEMENT (this “Agreement”) is made and entered into on the 1st day of August, 2019 (“Commencement Date”) by and between Fairview Health Services (“Fairview”), a Minnesota nonprofit corporation, and Independent School District No. 912 (“District”), contracting on behalf of Milaca High School (“School”) for a two-year period, the 2019-2020 and 2020-2021 school years (the “Term”). Fairview, District, and School may be referred to throughout this Agreement in singular as “party” and collectively as “parties”.
