This Audiologist Services Agreement (“Agreement”) is entered into by and between the County of Santa Clara, a political subdivision of the State of California d.b.a. County of Santa Clara Health System (“County”) and NAME (“Contractor”). This Agreement shall have an effective date of DATE (“Effective Date”). Audiologist and County may, from time to time, be referred to in this Agreement individually as either “Party” or collectively as “Parties.”
