Via this agreement and the attached annexures (“Agreement”), and subject to the terms of this Agreement, [name and address of the party 1 ](“Party 1”) and [name and address of the party 2 ](“Party 2”), do hereby authorize MaxMax Chambers to appoint a Mediator and a Case Manager to amicably resolve a dispute between the Parties.
