THIS AGREEMENT (hereinafter the “Contract” or the “Agreement”) is made this XX day of FEBRUARY 2014, by and between the People of the State of New York, acting by and through the Commissioner of the Office of General Services (OGS), whose office is on the 41st Floor, Tower Building, Governor Nelson A. Rockefeller Empire State Plaza, Albany, New York 12242 (hereinafter referred to as the “State” or “OGS”) and Healthcare Equipment and Parts Co. Inc. d/b/a Med-Part having its principal place of business at 1901 10th Avenue, Brooklyn NY 11215, herein after referred to as the “Contractor”). OGS and the Contractor are collectively referred to as the “Parties.”
