This Business Associate Agreement (this "Agreement") is entered into by and between the member of the New York State Society for Clinical Social Work, Inc. (“the Society”) who has renewed his or her dues in the Society [by signing and sending his or her check for his or her dues to the Society or by signing the dues form to authorize his or her charging his or her dues to his or her credit card, and not opting out of participating in the Society’s Business Associate Agreement which would enable him or her to receive consultation from the Society’s Committee on Ethics and Professional Standards with regard to certain issues where Protected Health Information would need to be shared with the Chairperson or other member of the Committee on Ethics and Professional Standards] or a member who has newly joined the Society [and has not checked the opt out box on his or her membership application form opting out of participating in the Society’s Business Associate Agreement which would enable h
