This Wishmakers On Campus License Agreement will not become effective unless and until it is approved by Make-A-Wish, as evidenced by the signature of an authorized Make-A-Wish representative on the back side of this form. If approved, we will send you a fully signed copy of this form, at which time your license to use the Make-A-Wish name and Marks will become effective.
