This PHYSICIAN-IN-TRAINING AGREEMENT (“Agreement”) is entered into by and between CEDARS-SINAI MEDICAL CENTER, a California nonprofit public benefit corporation (“Medical Center”), and , [M.D.] [D.O.] [D.D.S.] (“Physician-in-Training”), and establishes the Medical Center’s agreement to accept the Physician-in-Training into one of the Medical Center’s teaching programs, on the following terms and conditions:
