This form is for practitioners who are currently registered for an HBF Medical Gap agreement or as a VMO (VMO) and want to change their Gap Choice (Gap Choice). If you wish to register any new provider locations, please complete the Practitioner Application form. This form can be completed online by typing in the fields below. The completed form can be returned by email to medicalgap@hbf.com.au.
