The New York State Energy Research and Development Authority’s (NYSERDA) Charge Ready NY 2.0 program provides incentives to electric vehicle (EV) charging equipment owners who install eligible Level 2 charging stations at public facilities located within a disadvantaged community (DAC), workplace, and multifamily parking sites in New York State. NYSERDA will provide the incentives upon completion of the installation of eligible charging equipment and the provision of appropriate documentation. To participate in the program, Charge Ready NY 2.0 equipment owners must provide authorization from the property owner or an authorized agent of the property owner that gives the equipment owner permission to install charging equipment at the site. To authorize this installation, the property owner (or an authorized agent of the property owner) and the equipment owner must read and complete this form. If the equipment owner is the property owner, its completion of this form serves as attestation
