CHEERLEADING AGREEMENT FORMCheerleading Agreement • September 25th, 2020
I, (parent/guardian) and (cheerleader) understand and agree to pay all expenses outlined in the packet. I understand that my/my child’s violations of any school policies can result in my child’s immediate dismissal from the squad or other penalties as deemed necessary by the coaching staff and/or administration. If dismissed, I/my child will not receive any refunds. I also understand that if dismissed and/or quit, I/my child will forgo the right to participate in any and all remaining cheerleading activities for the remaining of the season as well as the cheerleading season that follows. Furthermore, I understand there will NO REFUNDS for monies paid to SLAM! Boca Raton for Cheerleading. I understand the financial commitment I am making, and understand these policies. Failure to comply with a cheerleading-related financial obligations to SLAM! Boca Raton in a timely fashion and/or according to any payment arrangement made with the SLAM!
