The City of Austin (“City”) and (“Contractor”) hereby agree that the following terms and conditions are made a part of the Agreement, to go into effect on July 30, 2020 (such contract and this Exhibit are collectively referred to herein as “Contract”), for all purposes. The parties acknowledge that this is required by the federal Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).

This Contract is between JM Engineering, LLC having offices at 2013 Wells Branch Parkway, Suite 117, Austin, TX 78728 and the City, a home-rule municipality incorporated by the State of Texas, and is effective as of the date executed by the City (“Effective Date”).
