THIS Contract Number N1000009046 for Claims Administration, Cost Management Services and a Preferred Provider Organization (PPO) Network for the self-insured PPO Health Plans and Management and Attorney Dental Plan, hereinafter referred to as (“Contract”) is effective January 1, 2008 by and between the County of Orange, a political subdivision of the State of California, hereinafter referred to as “County” and California Physicians’ Service, Inc. dba Blue Shield of California, with its principal place of business in San Francisco, California, hereinafter referred to as “Contractor”, which are sometimes individually referred to as “Party”, or collectively referred to as “Parties”.
