Kansas, Suite 1900, El Paso, Texas 79901. This CHR Services Agreement constitutes a Service Level Agreement under, and is issued pursuant and subject to the terms of, that certain Master Services Agreement dated as of , 2015_(the “Master Services Agreement”) between Provider and PdN HIE. Capitalized terms used herein without definition shall have the meanings ascribed to them in the Master Services Agreement.
