This Community Services Block Grant Award Agreement (this “Agreement”), entered into by and between the Indiana Housing and Community Development Authority (hereinafter referred to as “IHCDA”), and «Legal_Name» (hereinafter referred to as “Sub-grantee”) having a Unique Entity ID (“UEI”) of «New UEI_», and a service area consisting of (the “Service Area”) is executed pursuant to the terms and conditions set forth herein.
