This Direct Primary Care Program Agreement (“DPC Program Agreement”), dated , (“Effective Date”) is between The Schneider Organization Incorporated, d/b/a Concierge Health Network (“CHN”), and the entity listed in the table below (“Employer”). CHN and Employer are referred to herein, individually, as a “Party” and, collectively, as the “Parties.”
