This Agreement creates a retainer relationship between you and the Practice. The Practice will provide the services described below primarily through Dr. Edward A. Espinosa, DO, MPH. Under this agreement, you, the Participant, will obtain quality medical care with meaningful access to Dr. Edward Espinosa including the amenities listed in Exhibit A of this Agreement. Dr. Espinosa reserves the right to make changes to these amenities at any time in response to market conditions, regulatory requirements, and clinical developments. If such a change becomes necessary or desirable, Dr. Espinosa will send you an updated version of the contract.
