Terms of Services This concierge membership agreement is by and be- tween the undersigned member and Colleen Walker, M.D., PLLC. The terms and conditions of this agreement are outlined below. The parties have read and agree to fully comply with the terms as outlined in the agree- ment in its entirety. This membership agreement specifies the terms and conditions under which you the undersigned member (“Member”) may participate in the Ultimate M.D. program (“Program”). This program will become effective on the date you sign the agreement or the date in which the provider commences his/her Ultimate M.D. affiliated practice, which- ever comes later. Individuals are not guaranteed a membership and Colleen Walker, M.D., PLLC. reserves the right to decline membership. All indi- viduals must be approved and awarded a membership to join.
