This Conduit and Fiber Maintenance and Repair Services Agreement (including this Cover Page, the General Terms, and all Service Schedules and Orders, collectively, this “Agreement”) is signed by duly authorized representatives of the parties and effective as of the last date signed below on this Cover Page (“Effective Date”).
