FY 2025 CONSOLIDATED AGREEMENT

This Consolidated Agreement is made between the North Carolina Department of Health and Human Services, Division of Child and Family Well-Being (hereinafter referred to as “DCFW”) and Division of Public Health (hereinafter referred to as “DPH”), (herein DCFW and DPH collectively referred to as “NCDHHS”), and the Hoke County Health Department (herein after referred to as “LHD”) (herein NCDCFW, NCDPH, and LHD may individually be referred to as a “party” and collectively as the “parties”) for the purposes of maintaining and promoting the advancement of public health in North Carolina. This Consolidated Agreement shall cover a period from June 1, 2024 to May 31, 2025 and shall remain in force until the next Fiscal Year Consolidated Agreement is signed except as provided for in Section X. Provision of Termination.

This Agreement shall cover a period from July 1, 2019 to June 30, 2020 and shall remain in force until the next Fiscal Year Agreement is signed except as provided for in Section J. Termination.
