This MASTER PARTICIPATION AGREEMENT (“Agreement”) is by and between ____________ (“Participating Site”) and THE NEUROFIBROMATOSIS RESEARCH INITIATIVE (“NFRI”) directed by Dr. David Miller at Boston Children’s Hospital. Throughout this Agreement, the parties will be referred to as “___________” and “NFRI”. This Agreement shall become effective as of __________ (the “Effective Date”).
