This Agreement ("Agreement") is made and entered into as of the date last signed below (“Effective Date”), by and between the City of Chino Hills, a general law city and California Municipal Corporation ("City"), and Jacquelyn Crawford, an independent contractor doing business as ("dba") Mad Science Of North Orange County ("Contractor") to provide the classes or programs in science ("Class" or "Program") hereby agreed upon, as scheduled and described in this Agreement.
