This Contract is between the Department of Health Services (“Department”), acting in support of the Managed Care Organizations providing program delivery services for the Family Care and Family Care Partnership programs, and ("Contractor"), effective on 02/26/2010 (“Effective Date”). The Contract period will run for five (5) years from the Effective Date with an option to renew for two (2) additional two-year periods upon mutual agreement of the Parties.
