This Contributors Agreement (the “Agreement”) is made as effective as of , 20 between the American Society for Health Care Risk Management a Professional Membership Group of the American Hospital Association (“AHA” or “ASHRM”), an Illinois not-for-profit company and (“Contributor”) regarding Contributor’s original content, and any and all parts, portions or elements thereof, submitted to ASHRM at any time (the “Content”) to be used for the ASHRM FORUM (the “Project”).
