This Cost-Plus Addendum (“Addendum”) amends and is incorporated into and made a part of the Group Contract(s) entered into by and between Blue Cross and Blue Shield of Kansas City, on behalf of itself and its subsidiary, Good Health HMO, Inc., d/b/a Blue-Care, if applicable (collectively, “BCBSKC”) and SHAWNEE MISSION USD 512 (“Employer”). This Addendum shall be effective January 1, 2025 (the “Effective Date”).
