I, the parent of , acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Lord of Life EEC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Lord of Life EEC cannot guarantee that my child will not become infected with the Coronavirus/ Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other students and their families.

During the COVID 19 health pandemic, we have taken extra precautions to protect your child/children while at The Dance Center of Spokane, including but not limited to: deep cleaning each day, limiting class sizes, staff to wear masks as required/mandated by the state of Washington, persons entering the building are required to wear masks as required/mandated by the state of Washington, extra classroom cleanings, and plenty of hand washing.

IN CONSIDERATION for myself and/or my children listed above being permitted to utilize the services, utilize the facilities and/or participate in the programs of the Prince William Lassie League (the PWLL), including, but not limited to, observation or use of facilities or equipment, or participation in or acting as a spectator during any program affiliated with the PWLL, the undersigned, on behalf of himself or herself and such participating children and any personal representatives, heirs, and next of kin (hereinafter referred to as "the undersigned") hereby acknowledges, agrees and represents that he or she has inspected and carefully considered such premises, equipment, and facilities and has considered the PWLL’s programs and that the undersigned finds and accepts same as being safe and reasonably suited for the use or participation by the undersigned and such participating children.
