forth in Section 12.S of this Agreement (the “Effective Date”) by and between California Physicians’ Service d/b/a Blue Shield of California, a California not for profit mutual benefit corporation, located at 601 12th Street, Oakland, California 94607, (“TPA”) in its capacity as a Third Party Administrator contracted with the California Government Operations Agency (“Agency”) for the purpose of administering the Vaccine Provider network and related services in connection with the State of California COVID-19 Vaccination Program (“Program”) and (“County”) on behalf of itself, its agencies, and its owned and operated hospitals, medical clinics, and vaccination sites and other similar entities providing vaccines and vaccine-related functions identified on Exhibit B attached hereto and by reference incorporated herein (collectively, “Vaccine Provider’).
