Schedules and Statements of Work, the “Agreement”) is made, entered into and effective as of the date set forth in Section 13.S of this Agreement (the “Effective Date”) by and between California Physicians’ Service d/b/a Blue Shield of California, a California not for profit mutual benefit corporation, located at 601 12th Street, Oakland, California 94607, (“TPA”) in its capacity as a Third Party Administrator contracted with the California Government Operations Agency (“Agency”) for the purpose of administering the Vaccine Provider network and related services in connection with the State of California COVID-19 Vaccination Program (“Program”) and (“Provider”) on behalf of itself, its subsidiaries, its vaccination sites and other similar entities providing vaccinations and vaccination-related functions, and its subcontractors engaged by Provider to perform obligations pursuant to this Agreement, all of which are identified on Exhibit D attached hereto and by reference incorporated here
