You agree you have read and agree to the terms and conditions of the Account. You hereby make application for membership in MAX Credit Union and agree to conform to its by-laws and amendments thereof, copies of which have been made available to you and subscribe for at least one (1) share. You certify that: You are within the field of membership for this Credit Union; the information provided on this application is true and correct; and your signature on this card applies to all accounts under you name at this Credit Union. You also agree to be bound to the terms and conditions of any account that you have in the Credit Union now or in the future.
