THIS MASTER PARTICIPATION AGREEMENT (“MPA” or “Agreement”) is made as of June 3, 2019 (the “Effective Date”), by and between Hamilton Health Sciences Corporation, a corporation incorporated under the Corporations Act (Ontario) and operating CritiCall Ontario out of 1725 Upper James Street, Suite 200, Hamilton, Ontario L9B 1K7 (“HHS”), and , a corporation incorporated under the Corporations Act (Ontario) that is a public hospital under the Public Hospitals Act (Ontario) (the “Hospital”), operating out of , (each a “Party” and together, the “Parties).
