By signing this form, you give permission for culinary teachers to contact you should there be a question or concern involving food allergies or sensitivities. On this form, please list any and all food substances that your child is NOT allowed to eat due to a religious or physical explanation.

Join 4KIDS for an evening of local food fare, where guests will taste delicious lite bites and sweet treats from various 4KIDS' culinary friends across South Florida. The Culinary Takeoff event has an approximate attendance of 300 community members, businesses & church partners coming together to raise funds & create awareness for the children & families that 4KIDS serves.
