The Commonwealth of Virginia Data Trust Member Agreement (the "Agreement") is entered into on MONTH DD, YYYY ("Effective Date") by and between the undersigned (hereinafter referred to individually as "Data Trust Member" and collectively as “Data Trust Members”), and the Chief Data Officer for the Commonwealth of Virginia ("Trustee"), on behalf of the Virginia Commonwealth Data Trust ("Data Trust"). Data Trust Member and the Trustee may each individually be referred to herein as a "Party" and collectively as the "Parties."
