This Commercial Debit Cardholder Agreement (the “Agreement”) outlines the terms, conditions and disclosures for your Visa-branded debit card (“Card”) and the associated transaction account (“Transaction Account”) issued to you by Patriot Bank, N.A. (the “Bank”), Member FDIC, pursuant to license by Visa U.S.A. Inc. The Bank has retained Apto Payments Inc. (the “Servicer”) to provide certain banking and cardholder support services on the Bank’s behalf (the “Services”) using the Servicer’s card program management technology (the “Servicer Platform”). Servicer will make the Transaction Account and the Card available to you by integrating the website or mobile application of the company whose logo appears on the Card (the “Program Partner”) with the Servicer Platform. Please read this Sequence VISA Debit Cardholder Agreement (the “Agreement”) carefully and retain it for your future reference. This Agreement applies to the Sequence VISA Debit Card (singularly or collectively, the “Card”), which has been made available to you by Lance Global Inc dba Sequence (“Program Partner”), the program partner responsible for managing the Card program, in partnership with Thread Bank, a bank chartered under the laws of the State of Tennessee (the “Bank”), a member of the Federal Deposit Insurance Corporation (“FDIC”). The terms and conditions of this Agreement apply to all Cards issued to you by us, all replacement Cards that we may issue to you from time to time at our option, and all transactions resulting from the use of the Card through a point-of-sale (“POS”) device or any other use of the Card, whether electronic or otherwise.
