This Agreement was made under the supervision of Supported Decision-Making New York (SDMNY), a project funded by the New York State Developmental Disabilities Planning Council (DDPC). (See "Attachment A.") The undersigned "Decision-Maker" and "Supporter(s)" have completed a professionally evaluated facilitation process that began on [insert date facilitation began] and concluded with the signing of this Agreement. A facilitator trained and supervised by SDMNY assisted the parties in negotiating and fully understanding the terms of this Agreement.

My supporter(s) may also assist me in facilitating decisions regarding my day-to-day health, safety, welfare, or financial affairs by providing me with the most up-to-date and relevant information available and known to my supporter(s). My supporter(s) does(do) not make decisions on my behalf and shall not coerce or manipulate me into making a decision. The decision that is made will be my own final and ultimate decision. Cash or kind will not be exchanged for assistance provided pursuant to this agreement.
