If there will be one or more nominated persons appointed to assist with managing the Direct Payment then the Direct Payment recipient must sign below to say they understand that they retain legal responsibility for any actions taken by the nominated person in relation to the Direct Payment. The nominated person(s) must also sign to say they understand the terms and conditions of the agreement.
