When the form is complete and signed by the student, instructor and approved by the chair, the Dance office will then provide enrollment instructions. The student must enroll in the course.

Students may elect to take a maximum of one half-credit fourth year undergraduate course which has been revised for graduate credit. Students, who wish to pursue this option, should first browse the Undergraduate Calendar and timetable to identify fourth year courses relevant to their interests. The student should then approach the course instructor to determine if the course requirements could be modified to make it a graduate-level course. Once a Faculty Member has agreed s/he and the student should fill out this form including readings, assignments, meetings, evaluation and due dates, and submit it to the Graduate Program Director for approval. A copy of this agreement is kept in the student's file. Students should initiate this process well before the start of the term. Graduate students may only take one half-credit course in which the majority of students are undergraduate students.
