This DSRIP Master Services Agreement (this “Agreement”), is made and entered into as of the latest date indicated on the signature page, by and among xxxxxxxxxxxx (“Participant”), New York City Health and Hospitals Corporation (“HHC”), and HHC Assistance Corporation d/b/a OneCity Health Services, a not-for-profit corporation in which HHC is the sole member (“CSO”), in connection with the Delivery System Reform Incentive Payment (“DSRIP”) Program Performing Provider System led by HHC. HHC, CSO, and Participant are referred to each as a “Party” and collectively as the “Parties.” Unless otherwise defined below, capitalized terms used herein have the meanings set forth in Exhibit 1 (Definitions).
