Bellefontaine City Schools is allowing the use of an electronic device to you during the current school year. In order to use an electronic device each day, you must sign this Electronic Device Loan Agreement to indicate that you understand, accept, and agree to do the following:

The device identified above along with Charger, cables and Carrying Case is being loaned to Student and is in good working order unless otherwise indicated. Student acknowledges the responsibility to properly care for the equipment and ensure that it is retained in a safe environment. A $75.00 rental fee is assessed for use of the device for grades 6-12. A $25.00 rental fee is assessed for use of the device for grades 4-5.

I, the under signed, understand and agree that the electronic device(s) and its included items listed below are property of Floyd County Public Schools (FCPS) and are in my possession and responsibility until the end of the school year. I further understand I will accept responsibility for any damages or losses that occur to the electronic device(s) while it is in my possession and will make reparation so losses can be recovered.

This equipment is, and at all times remains, the property of West Holt Public Schools of Atkinson, Nebraska, and is herewith lent to the student for educational purposes only for the Academic School year. The student may not deface or destroy this property in any way. Inappropriate use of the machine may result in the student losing his/her right to use this computer. The equipment will be returned to the school when requested by West Holt Public Schools. If the student withdraws from West Holt Public Schools prior to the end of the school year the laptop will need to be returned prior to the last day of attendance.
