This AGREEMENT is made on this day of , 2003, in Johnson County, Kansas, by and between the City of Overland Park, Kansas, hereinafter called the "City" and CorporateCare Occupational Medicine Network, hereinafter called "Contractor." It is the intention of the City to secure services relating to the City’s Employee Wellness Plan. This Plan is for the benefit of employees, retirees, governing body members, and their families, hereinafter called "Participants."
