This Agreement describes the arrangement between the Group and a). Tufts Associated Health Maintenance Organization, Inc. for services provided in relation to the fully-insured benefit plan underwritten by Tufts Associated Health Maintenance Organization, Inc., or b). Tufts Insurance Company, for services provided in relation to the fully-insured benefit plan underwritten by Tufts Insurance Company (hereinafter referred to as “Tufts Health Plan”) for services provided in relation to the fully-insured benefit plan underwritten by Tufts Health Plan. The Group is an Eligible Small Business as defined in M.G.L. c.176J and has 9 or fewer Eligible Employees as defined in the statute.
