This Enhanced Care Management Provider Services Agreement (the “Agreement”) is entered into this 1st day of January, 2022, by Partnership HealthPlan of California, a public entity referred to as (“PARTNERSHIP” or “PLAN”) and between County of Marin Health and Human Services by and on behalf of its providers either employed and/or contracted (collectively, “Provider” or “Provider Group”). PARTNERSHIP and Provider are collectively referred to as the “Parties” and individually as a “Party.”
